Unum Life and AD&D - company paid
All full-time/regular teammates (US locations only) are automatically enrolled in a company paid Life and AD&D insurance policy through UNUM. The policy coverage amount is 1x your annual salary ($50,000 minimum / $200,000 maximum). You'll have the opportunity to designate your beneficiary when you complete your initial benefits enrollment, or anytime thereafter using the Benefits worklet in Workday. Please consult our Life and AD&D Summary for more information on coverage.
Note - The guaranteed issue amount above is only applicable to New Hires and for the Open Enrollment 1/1/23 effective date. Any changes made outside of these events will require an EOI.
UNUM Optional Life and AD&D - TEAMMATE PAID
You may also choose to purchase additional coverage for yourself, your spouse, and/or children, domestic partner, domestic partner and/or children. Please note, you must enroll in your own optional coverage in order to add dependent coverage. Your dependents' combined coverage may not exceed 100% of your optional coverage benefit amount. After age 65 your benefits may change, and upon retirement they will cease. Please consult our Optional Plan Summary and Certificate for more information on coverage.
Yourself:
You may purchase coverage in an amount from $1,000 to $500,000 in $10,000 increments up to 5x your annual earnings.
For coverage amounts exceeding $150,000, Workday will trigger a To-Do notification with a Single Sign-On link to complete a Statement of Health. This will appear in your Workday inbox. The Statement of Health is a questionnaire UNUM asks to establish Evidence of Insurability. At the end of the process, UNUM will show a decision that will update Workday in real time.
If you do not complete this task within 60 days, your election of optional coverage will not be covered at the elected amount.
Your Spouse/Domestic Partner:
You may purchase coverage for your spouse or domestic partner in increments of $5,000 up to $500,000.
Spouse coverage can’t exceed 100% of the coverage amount you purchase for yourself
Your spouse can get up to $25,000 with no health questions. For coverage amounts exceeding $25,000, Workday will trigger a To-Do notification to complete a Statement of Health.
Your Children:
You may purchase coverage for your children aged 6 months to 26 years in increments of $2,000 up to $10,000.
The maximum benefit for children live birth to 6 months is $1,000.
