All full-time/regular teammates are automatically enrolled in a company paid UNUM Short Term Disability policy that covers a portion of your wages if you become sick or injured and cannot work. The STD benefit pays 60% of your weekly salary to a maximum of $3,500, on a tax free basis, after any applicable state benefits. UNUM Long Term Disability, POLICY#818852 01

All full-time/regular teammates are automatically enrolled in a company paid UNUM Long Term Disability policy. If you are sick or injured and cannot work beyond the 90 days of Short Term Disability, the LTD benefit pays 67% of your monthly salary to a maximum of $15,000, on a tax free basis.   • The Unum team will review the existing Short Term Disability claim 30 days before the end of the Short Term Disability to review/initiate a Long Term. The case manager will connect with the claimant directly to facilitate.
