Life Insurance Continuation Information 2020

2020 Anthem — If you are no longer with Specialized:

    • Option 1 - Covert your Coverage - document here

    • Option 2 - Port your Coverage - document here

  • We recommend you to contact Anthem for full details and explanations around your rights to either Convert or Port your coverage.  

    • Contact: 800-801-6142

  • Once you have chosen your option, the request form should be completed and returned to People & Culture within 20 days. People & Culture will complete the employer section and send back to you to mail to Anthem.

    • Note – Anthem must receive the completed form and your first premium payment within 31 days from your termination date. You will then be contacted by Anthem to manage your policy on an individual basis.
