2021 UNUM— If you are no longer with Specialized:

  • How can I continue my life insurance policy?

    • We recommend you to contact UNUM for full details and explanations around your rights to either Convert or Port your coverage.

  • How can I apply?

    • Once you have chosen your option, the request application should be completed and returned to HR-Ops within 20 days. Please send your form to HR-Ops who will complete the employer section and send back to you so you can mail to UNUM.

      • Important to Note – UNUM must receive the completed form and your first premium payment within 31 days from your loss of coverage date. You will then be contacted by UNUM to manage your policy on an individual basis.
