Company Paid Life Insurance

All full-time US teammates are automatically covered by company-paid Life and AD&D insurance through UNUM. Coverage is 1× your annual pay (minimum $50,000; maximum $200,000). You can name a beneficiary during your initial benefits enrollment or later in the Benefits worklet in Workday. See the Life and AD&D Summary for details.

Note: The guaranteed issue amounts above applied only to new hires and the Open Enrollment effective date 1/1/23. Any changes made outside those events require evidence of insurability (EOI).

Voluntary Life Insurance

You can buy extra life and AD&D insurance for yourself, your spouse/domestic partner, and your children. You must enroll in your own optional coverage to add dependents. You can enroll during your initial benefits selection or yearly open enrollment. Dependent coverage can’t exceed 100% of your optional amount. Benefits may change after age 65 and stop at retirement. See the Optional Plan Summary and Certificate for details.

You:

  • Buy $1,000 to $500,000 in $10,000 steps, up to 5× your annual pay.

  • If you choose more than $150,000, Workday will send a To‑Do to complete a Statement of Health (Evidence of Insurability). UNUM will decide and update Workday in real time. If not finished within 60 days, the extra amount won’t be covered.

  • You must enroll in at least $10,000 voluntary coverage during your initial enrollment period to guarantee up to $150,000 without a Statement of Health.

Spouse/Domestic Partner:

  • Buy in $5,000 steps up to $500,000.

  • Spouse coverage cannot exceed your coverage.

  • Up to $25,000 requires no health questions. More than $25,000 requires a Statement of Health via Workday.

Children:

  • Buy for ages 6 months to 26 years in $2,000 steps up to $10,000.

  • For newborns up to 6 months, the maximum is $1,000
