How Can I get a replacement or extra id card?

Medical

Teammates on our Anthem plans can visit Collective Health to request new medical ID cards. 

If you are a teammate with a Kaiser plan, you can contact them directly for help with your plan by calling (800) 464-4000 or visiting their member website

Dental

Call Guardian Dental for replacement ID cards: (888) 600-1600

Vision

EyeMed Vision for replacement ID cards: EyeMed Visions send 2 ID cards in the subscriber’s name when you join, but the card is not needed when you visit your eye doctor. If you lose your card or need extras for your family, you can print a replacement on the member portal

WHERE CAN I SEE HOW MUCH A CERTAIN INSURANCE PLAN WILL COST

Navigate to the Health Plan tab and select your state-specific page. From there, you can browse the different options and find the monthly costs (premiums) above the plan coverage details. There are 3 costs depending on how many people from your family you are insuring: Employee only, Employee +1, Employee + 2 or more.

 

What IS OPEN ENROLLMENT?

Open Enrollment is the only time period each year to add, drop or make changes to your insurance coverage effective for the following calendar year.

CAN I MAKE CHANGES TO MY PLAN ELECTIONS THROUGHOUT THE YEAR? 

Yes and No. You may only make changes when a qualifying life event occurs. For more information on Qualifying Life Events, please see the QLE page.

 

CAN I DROP MY INSURANCE IF I AM BEING ADDED TO MY SPOUSE’S PLAN? 

Yes - this falls under a Qualifying Life Event. The change needs to be done in Workday by choosing the benefit event type 'spouse/dependent loses/gains coverage'. You must submit changes within 30 days of the life event occurring. 

 

HOW CAN I ENROLL IN BENEFITS ON WORKDAY AS A NEW HIRE? 

Step 1: Go to your inbox in Workday (upper right corner)

Step 2: Before you can enroll, be sure that you completed the following tasks waiting for you in your Workday Inbox: emergency contacts, personal contact, W4 and I9. 

Step 3: Look for the 'Benefit Change - New Hire' action in your inbox and follow the steps. At the end of the process, you'll check the box to confirm your elections, and have a chance to print out your enrollment.

 

Who is considered to be a dependent?

A dependent is considered a legal spouse, domestic partner, birth child, adopted child, or guardianship of a child. Children up to the age of 26 are allowed to be enrolled on their parents insurance.


Life Insurance Coverage

 

Do I have life insurance?

All team members are automatically enrolled in our basic plan. We also offer a buy-up option with expanded benefits at an additional premium.

 

DO I HAVE TO HAVE A SSN FOR MY DEPENDENT/ CHILD?

You can add your child to your benefits without a social security number.  Just to be sure to update Workday with their SSN once you have received it.

 

HOW MUCH LIFE INSURANCE CAN MY SPOUSE HAVE?

Your spouse is not covered under the basic plan. However, Specialized does offer a buy up plan that covers your spouse at a maximum of $500,000, but cannot exceed the amount of coverage you have.

 

Can I name my child as a beneficiary? 

Naming minor beneficiaries is not recommended. Generally, insurance companies will not pay death benefits directly to minors. The benefits would likely be held until a court-approved guardian is named, which can take several months and usually requires the hiring of a lawyer. However, the employee may consider naming a trustee of a trust for a minor as beneficiary to help ensure competent management of the proceeds. By naming a trustee of a child’s trust as beneficiary, the proceeds would be managed by the trustee (person or institution).

401k

 

How can I roll over my existing 401k to MY Specialized 401K account?

Step 1: Log on to the Fidelity website

Step 2: Select “View Summary” and select the “Rollovers” tab

Step 3: Choose where you want to roll over your funds

Step 4: Follow the step-by-step instructions to provide your previous plan details, request a check, provide contribution information and send over your documents from your previous 401(k) provider.

 

Where can I view my 401k account?

You can view and manage your account from the Fidelity website.  You will need to create your own username and password when accessing your account for the first time. If you are looking for more information on matching/contributions to a 401k click here.

 

How do i change the amount of my contributions or elections?

All changes to your 401(k) can be made directly on Fidelity website here. The first time you visit their site, you'll need to create a separate login and password. 

 

Do I have to enroll right away or can I wait?

Short answer: It's up to you.  You can opt-out of making contributions when completing your new hire paperwork.  

 

How soon will my Fidelity contribution changes go into effect on my paycheck?

You will see the changes to your contributions in about 35 days.

How does Anthem verses EDD work?

For Californians: Anthem provides Specialized's short-term disability insurance. Anthem will assume that people looking for disability benefits will first file a claim with the state EDD for the 55% benefit. You may file for EDD and Anthem at the same time.

Those outside of California: If you are outside of CA, and your state doesn’t offer disability plans, Specialized offers the Anthem short term disability plan at 60%.  More information here.

Why was my FSA Claim denied?

There are 4 reasons that claims are denied:

1) The insurance carrier hasn’t paid their portion of the bill yet, and therefore FSA doesn’t know how much will be the team member portion.

2) You may be over the limit on your FSA Debit Card

3) Your account may be on hold due to an outstanding substantiation claim

4) You may have used all the money you have contributed to your FSA account

WHEN WILL MY FUNDS BE AVAILABLE?

For dependent care FSA, you will only be able to use the amount that you have actually contributed to your FSA.

For medical FSA, your entire contribution amount will be available at the beginning of the calendar year. You can spend your funds as quickly or slowly as you need to, and will continue to contribute throughout the year via payroll deduction until the total amount is met.

Your Navia FSA debit card can be used for both types of funds, and the correct account will automatically be used at your time of payment. 

 

How will this affect my taxes?

Every individual tax situation varies so it's best to seek professional tax assistance.  All FSA contributions are pre-tax.

 

How do I file a claim?

Navia will mail you an FSA debit card once you enroll. You can use this to pay for expenses.

If you do not have your card or are unable to use it, you can get reimbursed for eligible expensed. You can file claims online with Navia or download their app to upload your receipts.  Sign-up for online claim submission here.

 

I haven’t received my FSA Card

Cards are generally sent within 2-3 weeks of enrollment. After that, our benefits support team can issue you a new one. You can email them here.
